Upon completing the new patient registration form, email programs@lishcare.org with Patient’s Full Name, Date of Birth, Address and Reason for requesting appointment. Indicate you are a new patient in your email and you will receive a reply with an appointment time.
We will be using the Zoom platform for your virtual visit.
